In a separate edition "published by order of the Medical Council of the Royal Jennerian Society" and in a reprint of this on p. 104-105 of Charles Maclean's On the state of vaccination in 1810 (London, 1810) the author is identified as John Addington. The separate edition, Maclean states, "must have been published between 1803 and 1806."
